Completing the bereavement form
Please download, complete, save it, and email it to us with any relevant documents. We’ll then be in contact within 2 working days to let you know what happens next.
As well as providing existing UW details and account number on the first page, please also ensure that on page 3 you read the declaration and tick the authorisation box alongside your name and the date.
How to send back your completed form
Please send your completed form as an attachment via the 'Email us' option at the bottom of this page. In the drop down options, please select ‘Account management’ then ‘Bereavement Form Submission’ and continue filling in the rest of the required fields before attaching your form. Completing all the available fields will help us review things more quickly - so please provide as much detail as possible.
